Hearing Aid Purchase Quote Sheet
Here is a new Hearing Aid Purchase Quote sheet to use. It is to be given to the patient at the hearing aid evaluation. A signed copy is kept in the patient file. This is so that the patient can clearly and easily see the cost involved so that we can help reduce any confusion. You can download this form to your computer and use it from there. Each line can be edited to add additional descriptions or items that are not automatically present. Even if we are not charging for an item or description, it is important to list everything to help display the value of the services they are receiving. If something is missing or you have a question, let me know.
